What it does → who wears it

Effect: when enemies are Charmed (and likely Frightened too — confirm the second word on the tooltip), the wearer gains +2 Bardic Inspiration charges.

Verdict: this is Eila's, no contest. Bardic Inspiration is a bard-only resource — +2 charges do nothing for Greta, Minthara, or Endblade. And Eila is the one who applies the charm in the first place, so she triggers it and cashes the reward. It's a charm-powered Inspiration battery built for exactly her.

🔁Why it's nuts on Eila — the loop

Hypnotic Pattern → charms a whole pack → helm refunds +2 Bardic Inspiration
→ spend on Cutting Words + heal-triggers → enemies' saves drop → more get charmed → ↻

She pays for her control with her control. Cutting Words shaves enemy saving throws so more of them get charmed, which feeds more Inspiration back. It's self-sustaining — and it's a stronger version of the "+1 Bardic Inspiration" gear we'd been eyeing, because this is +2 and repeatable.

Anything that lands the right condition on an enemy sets off the helm. AOE sources are best — they charm several enemies at once. Below, marked by who in the party can cast it.

Hypnotic Pattern technically applies "Hypnotised," which bg3.wiki lists inside the Charmed status group — so it counts.

🟠 Frightened — works IF the second word is "Frightened"

Confirm the tooltip first. If it reads "Charmed or Frightened," all of these light it up too:

❌ Does NOT trigger it — common mix-ups Hold Person / Hold Monster = Paralyzed, not Charmed. Slow, Faerie Fire, Tasha's Hideous Laughter (Prone), and Sleep don't count either. Only the Charmed group (and Frightened, if the helm includes it) feeds the +2.

🎮Getting the most out of it

1 · Spend before you re-charm Bardic Inspiration has a max cap (3 → 4 → 5 by level). If you charm while already full, the +2 is wasted. So pour Inspiration out first (Cutting Words, buffs, heals), then cast Hypnotic Pattern to refill.

2 · Favour charm-control over other lockdowns With this helm on, reaching for Hypnotic Pattern over Hold/Slow isn't just control — it's resource generation. Charm a pack and you've effectively cast a free buff on yourself.

3 · It stacks with Font of Inspiration At level 5 her Inspiration refills on a short rest. Add the helm's combat refills on top and Eila basically never runs dry — Cutting Words every fight, all fight.

🔎Two things to read off the tooltip

The second condition — is it "Charmed or Frightened," or just Charmed? That decides whether the orange table above is live.
The trigger frequency — does it fire once per turn, once per cast, or per charmed enemy? And does it care who charmed them? If any ally charming an enemy counts, then Endblade's Hypnotic Pattern feeds Eila's Inspiration too. If it needs her to inflict it, she stays the charmer.
